WHY THE ACCIDENTS KEEP HAPPENING
Young puppies have small bladders and very little warning, and the garden is full of far more interesting things than weeing. Many only partly empty their bladder outside, or only realise they need to go once they're back in the warm, so the puddle by the back door is part of learning rather than a step backwards.
The other big one is smell. Ordinary cleaners can leave a scent your puppy can still find, and ammonia-based ones can even smell like wee to them, so the same rug keeps getting picked. An enzyme cleaner, and changing what that spot means, usually sorts it.
WHERE IT USUALLY GETS STUCK
Holding it all the way round a walk
Your puppy has learned the garden is the toilet, which is great, so now they wait until they're home. They need help learning the toilet can move with them.
Rain, wind and wet grass
Plenty of puppies stand at the back door in the rain and refuse to budge (and who can blame them?). A sheltered spot and calm, focused trips help in any weather.
Accidents when you're out of the room
Reliable while you're there, then a puddle when you're not. Freedom should grow as their reliability does, so gates and smaller spaces are your friend for now.
